With adult day health locations in both Pleasant Hill and Antioch, we provide a combination of medical, social, and therapy services for frail adults, while giving their caregivers respite.
MSSP provides social and health care case management in Conta Costa, Napa, and Solano counties for those 65 and older, who receive Medi-Cal benefits with no share of cost.
The CCT program helps adults in Contra Costa, Solano, and Sacramento counties who have been living in a skilled nursing facility for 90+ days transition back into their own home or community living.
To augment our programs that directly serve clients and participants, we actively work to support & provide resources for family caregivers. We offer a variety of events and support groups specifically for caregivers.

PEAS provides assessment, outreach, screening, brief intervention, case management, and prevention/early intervention for seniors 60+ who are experiencing problems in their daily lives & need support to prevent isolation and depression in Solano County.

The development and success of our agency is due in large part to volunteer and community support. We have a myriad of ways to get involved, whether its volunteering your time or starting an fundraising campaign.
Volunteers are highly valued by our staff and by our program participants. We would not be able to accomplish as much as we do without the generosity of the many individuals who currently volunteer at Choice in Aging.
