Join the Contra Costa Care Transitions Task Force (CCCTTF) for a day of interactive conversation and hands on learning from renowned leaders in reducing hospital readmissions through clinical interventions,
community partnerships, and linkage programs. If you work with older adults, high risk individuals, and anyone who is at risk for hospital readmission, this is a must attend event! Five (5) CEUs available (3 for RCFEs).

Who We Are: The Contra Costa Care Transitions Task Force (CCCTTF) is a group of health care and social service providers improving care transitions and reducing avoidable hospital readmissions.

Mission: To bring cohesion across the entire care continuum to ensure a high standard of care and patient safety.

We accomplish this goal by partnering with hospitals and community providers on care transition efforts to support county-wide process improvement.
