California Community Transitions

The California Community Transitions (CCT) project, is California’s version of the national Money Follows the Person (MFP) Program. CCT helps adults who have been living in a skilled nursing facility for longer than 90 days and are covered by Medi-Cal transition back to their home or to community living with the help of a Transition Coordinator.

Our Transition Coordinators work with a myriad of community services to find resources, make connections and ensure a safe changeover to independent living. Make your own decisions about where and with whom to live. Explore your choices!

FEATURES AND BENEFITS OF THE PROGRAM:

  • Transition Coordinator will explore living choices and explain available services, such as home and vehicle adaptation, assistive devices, transportation options, wheelchairs and other medical equipment, self care training, and getting a personal care attendant among others
  • Nursing visits
  • Help coordinating services and supports that are available in your home community
  • Assistance talking about the program with your physician, family and other trusted people
  • Aid in working with Medi-Cal California Disabled Program to allow you to continue working, and still receive Medi-Cal benefits

PROGRAM ELIGIBILITY:

This program is available free of charge to persons of any age who qualify. If you have been living in a Medi-Cal paid inpatient facility for at least 90 days and wish to return to community living you are eligible. You do not have to wait 3 months to apply!
