Background

Choice in Aging, formerly Rehabilitation Services of Northern California (RSNC), was established in 1949 as a private, nonprofit 501(c)(3) public benefit corporation. Choice in Aging has centers in Contra Costa County and serves more than six hundred people each year. The Choice in Aging Board of Directors are dedicated community volunteers who provide guidance, support, and their expertise to the organization.

Services

Our programs include Adult Day Health Care services for frail adults, elders, and persons with Alzheimer’s disease and related dementias. This program is designed for loved ones who can engage in a safe and supportive environment during the day, while still living at home with their family members. Adult Day Health Care services are licensed by the California Department of Public Health and certified by the California Department of Aging.

We are also proud to offer information and referral resources to the local community, as well as support services to caregivers and family members of frail elders and persons with Alzheimer’s and related dementias.

Programs

The Mt. Diablo Center in Pleasant Hill provides the therapies and sense of community to frail and disabled adults to increase and maintain their optimal level of physical and mental functioning with the goal of remaining independent and preventing premature placement into nursing homes and unnecessary hospitalizations.  The daily activities are offered in English, Russian and Farsi to better focus on the cultural sensitivities of our clients, and include a dedicated Alzheimer’s disease and late stage dementia program.

The Bedford Center is the only licensed adult day program serving eastern Contra Costa County located in Antioch. This program is designed to meet the needs of adults with chronic conditions who require assistance with the activities of daily living, but want to remain living independently. The program offers therapies and a safe and supportive environment for adults with Alzheimer’s disease and other related dementias, while providing their caregivers much needed respite.

The Multipurpose Senior Services Program (MSSP) is available to Medi-Cal eligible elders who wish to remain living independently. Services at no cost are provided to those who qualify. These services help to ensure a safe living environment.

California Community Transitions (CCT) partners with home and community-based services providers to help eligible Medi-Cal adults living in skilled nursing facilities return to community living.

Assisted Living Waiver (ALW) allows Medi-Cal eligible seniors and persons with disabilities who might otherwise require skilled nursing, to live safely in an Assisted Living community.
